Brief Description of Duties and Responsibilities
  1. Prepare the Property Acknowledgement Receipt, photocopy the supporting documents, and route for signature of end-user/accountable officer;
  2. Prepare the Inventory Custodianship Slip (Semi-Expendable -High Value and Semi-Expendable -High Value), photocopy the supporting documents, and route to end-user/accountable officer;
  3. Coordinate with the Property Officer in the preparation of Report on the Physical Count of Property, Plant and Equipment (RPCPPE), and Report on Semi-Expendable;
  4. Maintain and update weekly the Property Cards by posting all unrecorded acquisition/receipt, issue/transfer, repair and maintenance, and disposal of PPEs;
  5. Prepare the Requisition and Issue Slip (RIS), and Report on Supplies and Materials Issued (RSMI), and route to end-user for signature.;
  6. Act as administrative support staff in the conduct of annual physical count of PPE in accordance with the Annual Inventory Plan;
  7. Maintain the copy of all ISO-related and COA-related documents such as PAR, ICS, PC, RIS, RSMI, and other inventory forms;
  8. Develop a data management system for all documents related to PAR, ICS, PC, RIS and RSMI in coordination with the OD-PICTS;
  9. Submit a monthly soft copy of PAR, ICS, PC, RIS, RSMI to the Head, FAD-PSMS and FAD-Accounting Section for reconciliation purposes;
  10. Scan all copies of PAR and ICS and coordinate with the Project Leaders for their reference in the preparation of DOST-Form No. 12;
  11. Prepare the venue and all IT equipment to be used in the meeting with the Chief FAD, Inventory/Disposal Committee members and Head of FAD-PSMS in relation to Inventory and Disposal of PPE, and prepare the minutes of the meeting whenever as necessary;
  12. Perform weekly 5S in the Stock Room to ensure the cleanliness, safety, and orderly arrangement of supplies and unserviceable PPEs; and
  13. Perform other tasks as may be assigned from time to time.

  4. Authenticated/certified true copy of eligibility/ratings/license;
  5. Photocopy of Transcript of Records & Diploma;
  6. Performance Evaluation Rating in the last 2 rating periods or its equivalent, (if any);
  7. Photocopy of Training Certificates;
  8. NBI Clearance (if not available, submit the online booking slip if recommended to the position);
  9. Medical Certificate (if applicant is recommended to the position).
